This software allows an interaction between patients, rehabilitators and their families in a way that makes the work done simple, motivating and effective. It consists of several modules: manager of evaluations and results, manager of cognitive exercises, manager of physical exercises (exercise cards with drawings and explanation of the correct way to perform them, videos and photographs), agenda manager (sessions, treatment, medication, coordination between different professionals and family members), manager, application of support products. This software allows individualized programs aimed at the prevention and maintenance of physical and psychosocial capacities of a person according to their level of dependency.

The manager consists of several modules, all of them integrated and currently in application.

• Manager of Ratings and results.
• Manager of Cognitive Exercises.
• Physical Exercise Manager (exercise cards, videos and photographs).
• Manager Schedule treatment sessions, medication, coordination between different professionals and family members. The manager allows family members to continuously monitor the treatment of the patient with continuous communication with the therapist.
• Support product application manager. It allows monitoring of those patients who perform treatment at home

The possible places of application are socio-sanitary centers such as day centers, specialized clinics, associations and hospitals.
